On TennisForum Hazel appears 36 times and Heather 28 times.
I have no idea. On the web I have found nothing of useful.

In my opinion it's quite possible that Mrs. D Trentham's first name wasn't "Naomi", and it just appeared in the Blast when she was wrongly associated with another woman whose first name was already known.

Mainly for English results in the 1950s and early 60s? If that's the case it's probably my error and they should read Hazel.

Possible LKK, but I think she was a Naomi, as the British magazine from 1936 has a quote from her partner calling her "Mie"-a likely nickname for Naomi.

Yes, Hazel appears for English results from 1952 to 1963, and Heather from 1952 to 1965.
On the web Hazel is much more frequent than Heather. Only one time a player named Heather appears:
