To me, it is the look of a life of excess: too much alcohol, maybe some drugs mixed in, too much meat, fat in the diet, etc.

If you can imagine the picture of how he would look now, if he had been living with a lifestyle of staying fit and healthy, it would be much more youthful and vibrant.
